DCS'S NOTICE OF HEARING ON DEPENDENCY PETITION No. P1300JD201500014 (Honorable Anna C. Young) IN THE SUPERIOR COURT OF THE STATE OF ARIZONA IN AND FOR THE COUNTY OF YAVAPAI In the Matter of: ADDISON DAVID ARNESON d.o.b. 09/22/2006 Person(s) under 18 years of age. TO: HOLLY JOAN HOLLISTER and MARK DONALD ARNESON, parents and/or guardians of the above-named child. 1. The Department of Child Safety, (DCS or the Department), by and through undersigned counsel, has filed a Dependency Petition pursuant to Title 8, of the Arizona Revised Statutes, Rules 4.1 and 4.2 of the Arizona Rules of Civil Procedure; and Rule 48(D) of the Arizona Rules of Procedure for the Juvenile Court. 2. The Court has set a Publication Hearing on the 9th day of June, 2015 at 10:00 a.m., at the Yavapai County Superior Court, Juvenile Justice Center, 1100 Prescott Lakes Parkway, Prescott, Arizona 86301, before the Honorable Anna C. Young for the purpose of determining whether any parent or guardian named herein is contesting the allegations in the Petition. 3. You and your child are entitled to have an attorney present at the hearing. You may hire your own attorney or, if you cannot afford an attorney and want to be represented by an attorney, one may be appointed by the Court. 4. You have a right to appear as a party in this proceeding. You are advised that your failure to personally appear in court at the initial hearing, pretrial conference, status conference, or dependency adjudication, without good cause shown, may result in a finding that you have waived your legal rights and have admitted the allegations in the Petition. In addition, if you fail to appear, without good cause, the hearing may go forward in your absence and may result in an adjudication of dependency, termination of your parental rights or the establishment of a permanent guardianship based upon the record and the evidence presented to the court, as well as an order of paternity, custody, or change of custody in a consolidated family law matter and an order for child support if paternity has been established. 5. PUBLIC NOTICE The City of Prescott is seeking five (5) volunteers to serve on a newly established Art in Public Places Committee. Duties of the Board include but are not limited to implementing guidelines for acceptance of Public Art and making recommendations to the City Council on all matters pertaining to the acceptance of public art by providing review an input on proposed donations and public art projects, operating impacts, maintenance, location and appropriateness of art to be included in the public art collection. The committee consists of five members appointed by the Council with terms expiring and available for reappointment in March 2016. The board is solely advisory to the Council but plays a key role of implementing guidelines for the acceptance of Public Art. Of the five (5) volunteers, there is a preference for one member to be a Prescott museum professional; one member to be a local gallery owner; one member to be an arts educator or artist who has completed public art projects; one member to be a member of the Prescott Area Arts and Humanities Council and one member to be a community member and/or member with a background in history/historic reservation or interest or degree in art.
